Proctological examination

The proctological examination begins with a discussion of individual and family medical history, it is important to clarify with your doctor your eating and defecation habits, major changes in your body weight (gain or weight loss), your medication intake, whether you feel pain during defecation, whether you have felt a lump or lump, whether you feel itching or irritation in the area of the rectum, and whether you have noticed e blood or secretions on toilet paper or underwear. After that usually the first step is manual inspection (rectal digital examination, RDV). If necessary, a rectal endoscopy (rectoscopic examination) can also be performed, with the help of which the lower 15-centimeter section of the rectum can be viewed. The test takes a few minutes and causes only minimal discomfort. The above tests can be supplemented with x-rays and biopsy sampling.

What diseases does proctology deal with?

Hemorrhoids treatment:

The formation of hemorrhoids is analyst (rectal related) and perianal (around the anus) under the mucous membranes of regions (submucosa) vasodilation of running venous plexuses cause These are primarily due to the weakening of the surrounding connective tissue and muscle fibers. Accordingly, hemorrhoid treatment is also carried out within the framework of proctology.

Its formation is facilitated by difficult bowel movements (constipation), increased abdominal pressure, a sedentary lifestyle and pelvic venous congestion occurring during pregnancy. Hemorrhoid nodules bulge towards the cavity of the rectum (internal hemorrhoids) and the vein can expand from the inside to the outside, in this way they come in front of the rectum (external hemorrhoids).

Its most characteristic symptoms are pain, itching, rectal discharge, and bleeding.

During the private proctology appointment, the aim of hemorrhoid treatment is to eliminate complaints as soon as possible with conservative therapy, i.e. avoiding surgical intervention. In milder cases, a lasting effect can be achieved with suppositories, ointments, and pain relievers.

Treatment of thrombosed hemorrhoids:

If blood flow in dilated blood vessels decreases dramatically, the probability of thrombosis increases greatly (most often in external hemorrhoids). The resulting disease is painful, but it can be cured by surgery under local anesthesia.

Thrombotic hemorrhoids can be treated with a number of surgical solutions, the essence of which in all cases is the removal of dilated veins running under the rectal mucosa.

Coronary artery ringing:

Proctology by private order, during the procedure a clamping ring is placed on the inlet and outlet veins of the hemorrhoid. During hemorrhoid ringing, it is applied with a special device. Excluded from the circulation, the bound golden veins die due to compression and within a few days their remains fall together with the ring. Tissue death is followed by a scarring reaction that lasts 1-2 weeks. In this way, hemorrhoids disappear. The hemorrhoid ring procedure is painless because there are no nerve endings in the treated area.

Rectal fissure his treatment:

A rectal fissure typically develops in the pain-sensitive area of the rectal canal. It can be caused by rapidly emptying diarrhea or the emptying of hard stools with increasing pressure on the stomach.

B-otulinum toxin injection is an effective alternative to surgery for rectal fissures. Surgery is recommended for those who do not improve with B-otulinum toxin treatment. Rectal fissure is a common disease. It is associated with rather unpleasant symptoms and affects the quality of life.

What are the symptoms?

The most common symptom is a streak of blood in the stool and the unbearable, sharp pain associated with it. That is why local anesthesia is also required for the examination.

The B-otox treatment relieves spasm of the internal sphincter without causing long-term damage, so unpleasant complications, such as incontinence does not develop. The B-otox treatment used during private proctology is effective in 85-90% of cases. Its effect can be felt within a few hours and lasts an average of 3-4 months. During this time, the resulting crack can also heal. If the crack was deep and the healing time is longer, the treatment can be repeated. The procedure is simple, neither local anesthesia nor anesthesia is required. In case of failure, a surgical solution can be performed, which is performed under anesthesia.

Rectal tumor

Routine screening is recommended over the age of 50, and in the case of a family history, at an age 10 years earlier than when a family member was diagnosed. It is also important to mention that the Human Papilloma Virus can also cause cancerous changes in the epithelium around the anus. In addition to these, it is important to see a doctor at any age if you see blood in your stools, your habits change (diarrhea, constipation), you lose weight or you experience weight loss for no reason.

Eczema

The skin around the anus can also often be affected by eczema. Its symptoms are itching, redness and pain, but due to the sensitivity of the area, these can be worse than in the case of changes occurring in other parts of the body. Treatment is complicated by the fact that people often do not consult a doctor when symptoms appear, as well as the fact that due to its location, it is chronically exposed to irritation, even just for cleaning. An important role in its treatment is a proper diet, stress reduction, and the use of local anti-inflammatory creams and the prevention of sweating.

Bladder

Vesicular skin lesions are most often caused by Herpes Simplex Viruses, which are spread sexually by coming into contact with infected saliva, skin or active herpes. Small blisters filled with water-clear liquid form on the skin, the environment of which may be inflamed. After the blister ruptures, they heal with scabbing in about a week, but the virus hides in the body and can cause symptoms again later. With oral antiviral medicine, there is no cure, but the possibility of recurrence of symptoms and transmission of the infection can be reduced.

Wart

When diagnosing a wart caused by HPV, it is important to type the virus with a laboratory test. Considering this, it is possible to determine how often it is necessary to go back for a screening test. It is recommended to give the vaccine even if the patient is already confirmed to be infected with HPV, because although it is not against the type already carried, the other components of the vaccine provide protection against certain types in the event of a future infection. Warts are also extremely important to treat as they grow and spread. The use of brushes is not recommended due to their irritating effect and difficulty in application. They can be removed on an outpatient basis by freezing or laser. Control - since due to the presence of the virus, new warts can develop without the person becoming infected again - and the screening of the sexual partner is also absolutely necessary.

Rectal pain

There are many things that can cause rectal pain, so it is very important to find out the cause in terms of treatment. Even chronic irritation caused by a bicycle seat can be the cause of the pain. In the case of an artery, it only causes pain in a more advanced state, if it is thrombosed, inflamed, edematous, extensive or inverted. The source of the pain is not necessarily the rectum, but the change in its environment. These can be pelvic inflammation, abscesses and gynecological, prostate or bone tumors.

Abscess

Inflammation of the glands located at the border of the skin and mucous membrane causes the abscess to spread under the skin. Most often, it appears as a painful, red, tender swelling of the skin, which may feel firm and warm. You may have a fever. Abscesses that develop deeper under the skin are rarer and less symptomatic. A ruptured abscess requires antibiotic treatment and irrigation. If it does not come out, it must be surgically opened before this. In case of recurrence, it is necessary to culture the sample taken from the abscess in order to administer more targeted antibiotic therapy, as well as to rule out the fistula.

Fistula

If the abscess does not burst and is not surgically treated in time, a fistula may develop as a complication, which is a connection between the bowel cavity and the surface of the skin. Symptoms may include skin irritation, foul-smelling secretions, throbbing pain that may increase with exertion (coughing, defecation), inflamed and swollen rectum. During a proctological examination, the diagnosis is made with the naked eye, with a probe or, in more complicated cases, with an imaging procedure (Ultrasound, MRI). In all cases, its therapy is surgical. In simpler cases, the fistula can be healed by cutting the skin and muscle around it. In more complicated cases, it may be necessary to insert a seton - a thin tube - into the passage before the operation to drain the infected fluid. This may take several weeks. In some cases, the operation can be performed under local anesthesia, but anesthesia may be necessary.

Discharge

Discharge from the rectum or its surroundings can be an accompanying symptom of diseases. It can occur in inflammatory bowel diseases. Such is the leading symptom of Crohn's disease and Ulcerative Colitis, but none of them. In addition, in the case of fistulas secretion may leak through the passage, but this does not occur in all cases either, because the passage may close to the outside world. Very rarely, it can be a symptom of sexually transmitted diseases

Itch

It is a very common symptom that occurs in many diseases. Even more sensitive skin can cause itching. Many types of inflammatory processes can be accompanied by itching. These include fissures, Crohn's disease, ulcerative colitis. It is also common in dermatological pathologies: allergies, atopic dermatitis, contact dermatitis. Inadequate hygiene or incontinence, as well as consequential infections (rare) can also cause itching. There are cases when the origin cannot be clearly identified. In this case, the symptoms are more likely to return.

Condyloma screening and removal:

Condyloma is caused by the Human Papilloma Virus (HPV) infection caused by the anogenital region of the rectum and genitals warty lesion. The wart form develops due to the fact that the affected epithelial cells begin to multiply and eventually small warts located in groups emerge from the surface of the skin.

The infection is spread sexually, by skin contact, and similar lesions can develop in both sexes. The pathogen that comes into contact with the surface of the epithelium enters the skin through small wounds in the epithelium, and in this way the epithelial cells become infected. In the vast majority of cases, there is no discomfort, but rarely, burning, itchy pain may occur.

Determining the infection is difficult, because it can remain hidden for many months, the pathogen does not cause symptoms. Regular screening is extremely important because HPV infection is closely related to the development of cervical cancer. Infection can be prevented by timely vaccination.

Viral warts that have developed must always be treated to prevent permanent growth and expansion. After the proctological examination, several methods can be used for removal: removal by freezing with liquid nitrogen, laser technique, excision with a scalpel, brushing and burning with a cautery.

Removal of skin appendage/rectal lesion:

A benign tumors may appear around the anus in some cases, which result in a wart-like change, these are also called skin appendages. Rectal warts can also develop secondary to rectal fissures or hemorrhoids.

Anal nodules, rectal abscess (abscess means a cavity filled with pus), rectal fistula are considered rectal lesions.

During private proctology, removal can be done by freezing, liquid nitrogen, laser technique, excision with a scalpel, brushing and burning. The treatment of other benign cell proliferation that appears all over the body, mainly involving connective tissue, is determined by its location and the nature of the problem caused.

Fistula examination and surgery:

We call it a fistula (or fistula). a set of pathological changes, during which connections and connections are created between body cavities or organs, through which solid or liquid material travels abnormally or escapes to the outside world.

Complaints: it causes bloody, purulent discharge, pain, inflammation, and an unpleasant odor. The change in stool habit is proportional to the intensity of the complaints.

The examination of the fistula begins with a visit to its opening to the outside world, following its progress we can conclude the fistula location, to its inner opening. This can be carried out in practice either with an imaging procedure (x-ray) or, if justified by a co-morbidity, with colonoscopy. For fistulas that have existed for several months, surgical intervention is the solution. Outpatient care is sufficient for the removal of superficial fistulas, which is performed under local anesthesia. Before and after the intervention, it is worth ensuring looser stools, which can be achieved with a diet rich in fiber and liquid. The recovery time depends on the type of fistula treated (superficial, deep, branching...).
